Comments:
An unannounced monitoring inspection was conducted on 09/28/2022 from 10:00am to 1:00pm. There were 77 children ages 3 years old to age 5 years old supervised by 12 staff. The physical plant, outdoor playground area, programming, menus, 5 staff records, 5 children?s records, 2 medications with corresponding authorization records, emergency drills, and emergency supplies were reviewed. Children were observed participating in snack time, table toy activities, blocks, transitioning to a nature walk and outside play. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-60-A-8
Description: Based on review of 5 child records, the provider did not ensure they had a written care plan for each child with a diagnosed food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The record for Child #1 did not contain an allergy care plan for their physician diagnosed food allergy.

Plan of Correction: Child #1 no longer attends our facility. Any future children w/diagnosed food allergy will have an allergy care plan.

Standard #: 8VAC20-780-80-A
Description: Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.

Evidence: During inspection in the Monday-Friday Full Day Fours group, there were 12 children present, but the written record reflected there were 13 children present.

Plan of Correction: Teacher was spoken to and now maintains a correct daily attendance.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.

Evidence: The wooden playground divider that keeps the pea gravel in place, had a 6 inch area that was sharp and could snag a child?s skin.

Plan of Correction: Wooden divider has been repaired.

Standard #: 8VAC20-780-530-A-1
Description: Based on documentation review, the center did not have at least one staff member in each classroom or area where children are present that had obtained cardiopulmonary resuscitation (CPR) certification.

Evidence: Three of the 4 year old classrooms and one of the 3 year old classrooms did not have at least one staff member that had documentation of cardiopulmonary resuscitation (CPR) certification.

Plan of Correction: All lead teachers are now certified in CPR and First Aid.
