A monitoring inspection was initiated on 10/22/2022 and concluded on 10/22/2022. There were 19 children present with six staff supervising and a director. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and six staff records were reviewed. The inspection started at 11:30 am and concluded at 1:45 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-70 Description: Based on record reviews the center failed to ensure that the staff records contained all the required information.
Evidence:
1. Staff # 1-6 did not have documentation that reference checks were completed before employment.Plan of Correction: The director will ensure that when references are conducted they are documented.
Standard #: 8VAC20-780-210-A-4 Description: Based on record review the center failed to ensure that all staff meet qualifications to be a program leader/teacher.
Evidence:
1. Staff # 1, 3, 6 do not meet lead teacher qualifications. All three staff were working as lead teachers on 10/12/22.Plan of Correction: The director will ensure that staff meet qualifications for the positions they hold.
Standard #: 8VAC20-780-240-B Description: Based on record review the center failed to ensure that all staff complete orientation training prior to working alone with the children and no later than seven days of the date of assuming job responsibilities.
Evidence:
Staff # 1 (hire date 8/26/22), staff #3 (hire date 9/16/22) and staff #6 (hire date 9/10/22) do not have documentation on file that shows they have completed orientation training within the set time frames.Plan of Correction: The director will ensure that all staff receive orientation within the set time frames and it will be documented in the staff files.
