Technical Assistance:
Technical Assistance given and Licensing Standards were reviewed regarding: stable furniture, handwashing, and menu substitution.

Comments:
An unannounced renewal inspection was conducted on October 14, 2022 from 9:45am to approximately 12:45pm. There were 48 children present and 9 staff members. A sample size of 9 records were reviewed which included five children?s records and four staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during free play, arts and crafts, diapering, and infant feeding. First aid kits and emergency preparedness documents were observed. Transportation vehicles were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site director at the exit interview.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on observation and interview, it was determined that the center did not ensure that each center shall maintain and keep a separate record for each child enrolled which shall contain the work phone number of each parent who has custody.
Evidence:

1. Child #1 (date of enrollment 6/14/22) does not have their father?s work number in the record.
2. Child #2 (date of enrollment 8/29/22) does not have their father?s work number in the record.
3. Child #3 (date of enrollment 6/06/22) does not have both parents? work number in their record.
4. Staff #1 confirmed that Child #1, Child #2, and Child #3 do not have work phone numbers of their custodial parents in their record.

Plan of Correction: The center responded with the following: Staff #1 will obtain the work phone numbers for the children?s parents at pick up.

Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, it was determined that the center did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition.
Evidence:
1. In the playground, there was an empty storage shed that had sharp plastic pieces coming off the handle.This is in reach of children. This poses a pinching and injury hazard.
2. The storage shed in the playground is approximately 2 to 3 feet high in which children can open the shed and fall inside the shed or harm themselves. The lid of the storage shed automatically closes when the top of the shed is lifted. This storage shed is an injury and trapping hazard for children.
3. In the young infant classroom, the crib sheet for Child #4 was loose and not fitted to the mattress. This is a suffocation hazard.
4. Staff #1 confirmed that the empty storage was a trapping and injury hazard and will move it immediately so that it is not accessible to children.
5. Staff #1 and Staff #2 confirmed that the crib sheet for Child #4 is loose and poses as a suffocation hazard.

Plan of Correction: The center responded with the following: The storage shed was immediately moved during the time of the inspection to ensure safety. The children will not have access to the storage shed. The crib sheet will be form fitted for Child #4's crib.

Standard #: 8VAC20-780-280-B
Description: Based on observation, it was determined that the center did not ensure that hazardous substances such as cleaning materials be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the 3 year old classroom with 7 children present, the cabinet above the sink contains bottles of disinfectant cleaners that are unlocked and accessible to children.
2. In the 16 month to 24 month classroom with 5 children present, there is a bottle of yellow disinfectant on a shelf that was unlocked.

Plan of Correction: The center responded with the following: The disinfectant cleaners were immediately locked during the time of the inspection.

Standard #: 8VAC20-780-520-C
Description: Based on observation and interview, it was determined that the center did not ensure that if diaper ointment or cream is used, the following requirements shall be met: these products shall be in the original container and labeled with the child's name.
Evidence:
1. In the 16 month to 24 month classroom, there was an unlabeled tube of diaper cream on the diaper changing table.
2. The classroom teacher confirmed that the diaper cream tube was unlabeled.

Plan of Correction: The center responded with the following: Staff #1 confirmed that all diaper creams will be labeled with the child?s name.
