A monitoring inspection was conducted on 12/05/2023 between the hours of approximately 9:30 am and 10:30 am. There were 13 children present, ranging in ages from 3 years to 5 years, with 2 staff supervising. Children and staff were observed while tracing numbers and letters and playing with toys.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-60-A Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain the work phone number and place of employment of each parent who has custody.Plan of Correction: We obtained the information from the parents.
Standard #: 8VAC20-780-420-E-3 Description: Based on record review, the provider did not ensure to annually, review with the parent the emergency contact information required to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.

1) The review of the emergency information in the record for Child #3 was dated 11/18/2022.Plan of Correction: We obtained the information from the parents.
