Technical Assistance:
Discussed ensuring that a copy of the written list of allergies is maintained wherever the children are in care including the gym, cafeteria and playground.

Comments:
An unannounced monitoring inspection was conducted from 4:00 - 4:30 pm with staff. There were eight children, ranging from five years to ten years of age, present with two staff supervising. Children were observed completing homework, silent reading, washing hands preparing for and eating snack. Snack served today: whole grain chips, salsa and cheese dip. Two onsite staff records were reviewed at the time of the inspection along with the daily attendance record, emergency drill log, written list of food allergies, sensitivities and dietary restrictions, two on-site child records and the required posted information. Two staff records were reviewed off site at the Ron Rosner YMCA on 10/12/2022. It was reported that there was no medication on site but there was one staff member with current certification in Medication Administration Training (MAT). Description: Based on staff interview, the facility failed to inform all staff who work with children of children's allergies, sensitivities and dietary restrictions. Evidence: when asked, not all staff could state which children in the group had allergies, sensitivities and/or dietary restrictions.

Plan of Correction: Spoke with staff and informed of all allergies at site.

Standard #: 8VAC20-780-40-M
Description: Based on review of documentation, and an interview with staff, the facility failed to maintain a current written list of all children's allergies, sensitivities and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present. Evidence: The list posted in the classroom was not dated, not confidential, and did not include the same children on the list posted inside the classroom cabinet.

Plan of Correction: Informed staff to keep allergy list confidential. Moved posted list to inside of snack cabinet.

Standard #: 8VAC20-780-560-J
Description: Based on observation, the facility failed to sanitize tables as required before snack. Evidence: staff stated they had sanitized the tables before children arrived, however, children were observed sitting at the tables reading and completing homework before eating snack. The tables were not sanitized again, before use for snack.

Plan of Correction: Trained all staff on table cleaning procedures.
