Comments:
An unannounced monitoring inspection was conducted on 10/17/22 from 6:30am to 7:05am with program staff. There were three children in care, supervised by two staff. Children were observed doing a Fall craft with the staff members. Three on-site child records were reviewed; additional off-site documentation was requested by the Licensing Inspector to be scanned. Two on-site staff records were reviewed and off-site documentation was reviewed at the Ron Rosner office on 10/7/22. Staff reported that there was no medication for children on-site at this time, but the program has one staff with current certification in Medication Administration Training. The attendance and emergency drill log were reviewed. The first aid kit, flashlight, and battery operated radio were observed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 10/25/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on review of three staff records, the program did not obtain the results of a search of the central registry within 30 days of employment for two staff. Evidence: 1. The record for Staff #1, with an employment date of 8/29/22, did not contain the results of a search of the central registry. 2. The record for Staff #2, with an employment date of 8/18/22, contained central registry check results dated 10/4/22.

Plan of Correction: Manager will contact DSS if results are not received within 30 days.

Standard #: 8VAC20-780-50-A
Description: Based on observation, child records were not treated confidentially. Evidence: On 10/17/22 the Licensing Inspector observed that the child records were kept in the Parent Sign In and Out Binder that each parent uses daily. Any person with access to the Binder, which is kept out on a table at the entrance to the program, had access to the child records that contained confidential/highly sensitive information.

Plan of Correction: Files have been put into a staff binder kept in staff cabinet.

Standard #: 8VAC20-780-540-C
Description: Based on observation and interview with staff, the program did not ensure that all required items were in the first aid kit. Evidence: The first aid kit did not contain a working thermometer. After staff replaced the batteries, the thermometer would not turn on.

Plan of Correction: A new thermometer has been provided.
