Comments:
An unannounced monitoring inspection was conducted on October 12, 2022 from approximately 9:30 am to 12:15 pm. There were 63 children in attendance and a total of 14 staff. 10 staff records, and 10 children?s records were reviewed. All areas of the center were observed including classrooms, bathrooms, hallways, kitchen, and playground. The children were observed learning about the letter B, listening to stories, playing with table toys, practicing writing, getting diapers changed, playing outside, and eating lunch while the licensing inspector was on the premises.

The First Aid Kit had all components. The license, current inspection, and evacuation plan were posted and emergency numbers were posted.

Violations were cited as a result of this inspection.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 10/25/2022. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on a review of records, the center failed to obtain a physical for each child no later than 30 days after the first day of attendance. Evidence: The record for child #4, date of enrollment 08/08/2022, did not contain evidence of a physical exam.

Plan of Correction: Per the Director "Documentation of physical exams were requested by management on the day of the inspection. Management will obtain the physical examination within the required time frame for newly enrolled children. The director has received the physical for child #4."

Standard #: 8VAC20-780-160-A
Description: Based on review of staff records, the center failed to ensure that each staff member submits documentation of tuberculosis screening no later than the first day of employment. Evidence: The record for staff #6, date of employment 10/15/21, contained documentation of tuberculosis screening dated 10/22/21.

Plan of Correction: Per the Director "Staff member was onboarding and completing paperwork. She has not started working in the classroom with children. The screening was completed by the employee, but the nurse designee did not sign the screening until the evening of 10/22/21."

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that areas of the center were maintained in a safe condition. Evidence: The sand table on the preschool playground has a wire that is missing the outer protective covering and is sharp and frayed.

Plan of Correction: Per the Director "Maintenance was notified of the wire on the sand table, and it was attended to on the last inspection. This violation is the other sensory table and maintenance was immediately notified. The wires have been immediately removed."

Description: Based on review of staff records, the center failed to ensure that at least one staff in each classroom or area where children are present have current certification in CPR. Evidence: Classroom #2 had 8 infants and 2 staff. The record for staff #9, date of employment 05/24/21, did not have evidence of CPR certification. The record for staff #10, date of employment 8/2/17, had CPR certification that expired on 5/6/21.

Plan of Correction: Per the Director "Management's office is right outside the infant door and both managers are FA/CPR certified. All staff start their first day of employment with us after completing their online portion of FA/CPR. Management will ensure at least one staff member in all area have completed FA/CPR training. FA/CPR training has been scheduled for November 15, 2022."

Standard #: 8VAC20-780-530-A-2
Description: Based on review of staff records, the center failed to ensure that at least one staff in each classroom or area where children are present have current certification in First Aid.
