Comments:
An unannounced renewal inspection was conducted on October 21, 2022 from approximately 10:15 am to 11:50 am. There were 27 children in attendance and a total of 5 staff present. 3 staff records and 5 children?s records were reviewed. All areas of the center were observed including classrooms, bathrooms, hallways, and the kitchen. The children were observed playing outside, participating in circle time, listening to a story, and dancing to music, while the licensing inspector was on the premises.

The First Aid Kit had all components. The license, current inspection, and evacuation plan were posted and emergency numbers were posted.

Violations were cited as a result of this inspection.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 10/26/2022. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description: Based on records review, the center failed to ensure that a required criminal history
record report is not accepted
more than 90 days prior to the date of employment. Evidence: The record for staff #1, date of employment 9/27/22, contained a criminal history record report dated 5/5/22.

Description: Based on records review, the center failed to ensure that staff submit documentation of a negative tuberculosis screening at the time of employment or 30 days prior to employment and prior to coming into contact with children. Evidence: 1) The record for staff #1, date of employment 9/27/22, contained documentation of tuberculosis screening dated 10/3/21.

Description: Based on observation, the center failed to ensure that each restroom is equipped with soap, toilet paper, and disposable towels. Evidence: 1) The restroom in the preschool building did not contain soap or paper towels. 2) The restroom in the school-age building did not contain soap or paper towels.

Description: Based on observation, the center failed to ensure that climbing equipment used by preschool children is located over resilient surfacing. Evidence: The playground contains a slide that is approximately 3 feet tall and is directly on the bare ground without a resilient surface under or surrounding the fall zone. The preschool children were observed playing on the slide.

Description: Based on observation, the center failed to ensure that disposable diapers are disposed in a leak-proof or plastic-lined storage system that is foot operated. Evidence: The trash can near the diaper changing table had a foot operated lid that did not open with the foot pedal and had to be manually opened with the staff-members hand. The trash can did not have a plastic-liner. The trash can contained several dirty diapers.

Description: Based on interview, the center failed to ensure that a menu listing foods to be served for meals and snacks during the current one-week period is posted or given to parents and kept on file for one week at the center. Evidence: When asked, the Director stated that she updates the parents electronically what the menu will be each day and does not send it out for a one week period.
