Technical Assistance:
1. Recommend all child records be reviewed to ensure completion.

Comments:
Conducted an unannounced inspection at 2:50pm. Observed 31 children + 2 staff. Ratios + supervision were in compliance. Children were observed arriving to the program, washing hands, eating snack, celebrating a birthday, playing table games + video games, shooting pool, and drawing/coloring. Staff were actively engaged with the children + activities, and they greeted each child as they arrived. The program areas were well supplied with games + equipment for the children. Areas of non-compliance with standards + laws reviewed were found. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on records reviewed, it was determined that 2 of 2 children?s records did not include required information.

Evidence:

1. Child #6 ? There was no physical exam + immunizations on-file.

2. Child #7 ? There was no documentation of the child?s gender, home address, parent home/work information, authorized pick-up?s, addresses for emergency contacts, physician + health insurance information, physical exam + immunizations, signed agreement, ill-child policy, reporting communicable diseases, and emergency medical authorization.
a. There was a file folder containing one piece of notebook paper with handwritten information listing several family member names + phone numbers, and a note that the enrollment form would be turned in on 8/23/22.

Plan of Correction: On site supervisor will follow up with main office for copies of physicals + immunizations. On site supervisor will contact child's guardian for all necessary paperwork.

Standard #: 8VAC20-780-550-D
Description: Based on records reviewed, it was determined that a monthly emergency evacuation drill was not conducted. Evidence: There was no documentation that a drill was completed in September 2022.

Plan of Correction: On site supervisor will add calendar reminders monthly for emergency drills.
