Technical Assistance:
- Discussed medication forms. Fairfax County Public School medication authorization forms deo not authorize Lee District to administer medication.

- Discussed orientation requirements to include new topics required for all staff hired after 10/13/2022.

Comments:
An unannounced renewal inspection was conducted today from 10:40am-12:00pm. A portion of the inspection was conducted with county officials via email to obtain background check information. The physical plant, 7 staff and applicant records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, and eating lunch brought from home. There was an assortment of age appropriate materials for the children in care. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.036-B-1
Description: Based on record review, one applicant did not have a repeat criminal record check on file after five years.
Evidence:
Applicant 3's record contained results of a fingerprint criminal record check conducted on 7/26/2017.

Plan of Correction: Had applicant 3 complete a new fingerprint check. Updated form has been added to licensing book. Confirm dates in employee files match those in licensing book.

Standard #: 8VAC20-770-40-D-1-a
Description: Based on record review, two applicants did not have the results of a central registry check and one did not have the results of a sworn disclosure or affirmation.
Evidence:
1. Applicant 3's sworn disclosure was dated 10/19/2022.
2. Applicant 2 and 3's record did not contain the results of a central registry check.

Plan of Correction: 1. Had applicant 3 redo a new sworn disclosure, date was entered wrong, should have read 9/19/2022. 2. Working with HR to receive results. 3. ensure dates are correct.
4. Met with HR on new procedures for getting results.

Standard #: 8VAC20-770-60-C-2
Description: Based on record review, two staff did not have the results of a central registry background check.
Evidence:
Staff 2 and 3's record did not contain the results of a central registry background check with in 30 days of employment.

Plan of Correction: Working with HR to receive results of the central registry background checks. met with HR and youth services to develop a better procedure to receive results.

Standard #: 8VAC20-780-60-A-8
Description: Based on a review of documentation, one child with a diagnosed food allergy did not have a written care plan to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
Child 3 did not have a written allergy plan despite having a documented diagnosed food allergy.

Plan of Correction: A written allergy plan has been added to Child 3 emergency form and to the classroom allergy sheet. Reviewed procedures with staff in Nov. staff meeting.

Standard #: 8VAC20-820-120-E-2
Description: Based on observation, the findings of the most recent licensing inspection were not posted in the facility.
Evidence:
The findings from the center's 5/5/2022 inspection were not posted in the facility on the date of inspection.

Plan of Correction: Most recent licensing inspection has been posted on the parent board.
