A monitoring inspection was initiated on 11/7/22 and concluded on 11/7/22. There were 24 children present, ranging in ages from 5 months to five years, with 8 staff supervising and two administration. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three children?s records and four staff?s records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-780-60-A Description: Based on record review and interview, the center failed to ensure children's records contained all the required information.
Evidence:
1. The record for child 2 only contained one emergency contact that was not the parent or guardian. The second emergency contact listed was also listed as the child's guardian.
2. The director verified the second emergency contact was listed as the child's guardian.
3. The record for child 2 documents an allergy for eggs and almonds. The allergy care plan only contained information regarding the egg allergy.
4. The director stated the allergy for almonds is new and the allergy care plan has not been updated.Plan of Correction: An additional emergency contact will be obtained for child 2. In the future emergency contacts will be reviewed upon receipt.
The parents of child 2 will be advised to obtain a new allergy care plan for child 2. In the future allergy care plans will be obtained for all children's allergies.
Standard #: 8VAC20-780-560-J Description: Based on observation, the center failed to ensure tables were sanitized before eating.
Evidence:
In the turtle's classroom staff 3 failed to sanitize tables prior to serving lunch.Plan of Correction: Staff will be retrained on this standard.
