Description: Based on record review, the center did not obtain the results of a criminal history record search or request a search of the child abuse and neglect central registry from all states prior to hire in which staff have lived in the last 5 years.

Evidence: Staff #3 (Date of Hire: 9/8/2022) has lived in Louisiana within the last 5 years. The center did not obtain the results of a criminal history record search from Louisiana or request a search of the child abuse and neglect central registry from Louisiana prior to hire for Staff #3.

Plan of Correction: Waiting for Louisiana to send results. Called again 10/31/2022.

Standard #: 22.1-289.058
Description: Based on observation and staff interview, the center was not equipped with at least one carbon monoxide detector.

Evidence: There was no carbon monoxide detector at the center at the time of inspection.

Plan of Correction: Had on located on lower level, add on upstairs.

Standard #: 8VAC20-780-160-A
Description: Based on record review, the center did not obtain documentation of a negative tuberculosis screening at the time of employment for all staff.

Evidence: Staff #1 (Date of Hire: 10/24/2022) does not have a tuberculosis screening on file.

Plan of Correction: Doctor had not given results, we received on 11/9/22. Screening done before 10/24.

Standard #: 8VAC20-780-40-M
Description: Based on observation, the center did not maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions.

Evidence: There was no posted allergy list in the Teddy Bears classroom and Child #4 has a diagnosed food allergy to eggs.

Plan of Correction: Allergy list posted in every room immediately.

Description: Based on record review, the center has not had an annual fire inspection.

Evidence: The last fire inspection was completed 9/13/2021.

Plan of Correction: Church locating but inspectors came on 10/27.

Standard #: 8VAC20-780-260-B
Description: Based on record review, the center has not had an annual health inspection.

Evidence: The center has not had a health inspection within the last year. The most recent health inspection was completed 10/8/2019.

Plan of Correction: Church usually completes, called to have them come.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the Teddy Bears classroom there were two disinfectant sprays stored unlocked on the shelf above the changing table.
2. In the Elephants classroom there were cleaning supplies stored in the unlocked closet.
3. In the Panda classroom there was bleach spray stored on top of the shelf by the window.

Plan of Correction: Obtained closet locks; teaches told that all sprays must be locked and not just out of reach.
