Comments:
An unannounced 60 day follow up inspection was conducted on November 3, 2022 from approximately 4:00 pm to 4:45 pm. There were 17 children in attendance and a total of 4 staff present. 4 staff records and 3 children?s records were reviewed. All areas of the center were observed including cafeteria, bathrooms, and the playground. The children were observed eating snack while the licensing inspector was on the premises.

The license, current inspection, menu, daily schedule, and evacuation floor plan were posted. The first aid kit was inspected and found complete. The center had a working flashlight and battery operated radio. Evacuation drills were conducted with the most recent on 10/25/22.

Violations were cited as a result of this inspection.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 11/6/2022. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on records review, the center failed to maintain and keep at the center a separate record for each enrolled child that contained the child?s date of birth, the name and number of the child?s physician, and parent agreements. Evidence: Three children?s records were reviewed. The records did not contain the child?s date of birth, the name and number of the child?s physician, or the parent agreements.

Plan of Correction: Per the center "we have reached out to families to correct this. Moving forward we will make sure to have complete child files the 1st week of attendance."

Standard #: 8VAC20-780-70
Description: Based on interview, the center failed to ensure that the name, address, and telephone number of a person to be notified in an emergency was kept for each staff person at the center. Evidence: When asked the Director stated that she did not have 2 out of 4 staff complete the emergency contact forms with that information yet.

Plan of Correction: Per the center "we have corrected this and will make sure this occurs before staff's first day at site."
