Comments:
An unannounced on-site monitoring inspection was conducted on 11/7/2022. There were 58 children, ages 8 months-12 years, and 15 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, and tv time. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on review of four staff?s records, the center failed to ensure that all staff have lived
outside of Virginia in the past five years had the out of state backgrounds completed.

Evidence:

1. Staff # 2 was hired on 5/31/2022 and lived in a non NFF (national finger print file) state in the past five years. The requirement is for an out of state criminal back ground check.

Plan of Correction: The director was not aware of the out of state background and will make sure that it is obtained and all new staff will be monitored.

Standard #: 8VAC20-780-40-M
Description: Based on review of the current written list of all children?s allergies, sensitivities and dietary restrictions documented and the list shall be dated.

Evidence:
The list that was kept in the infant room did not contain a date.

Plan of Correction: The list will be dated today and will be updated as necessary and dated.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that all areas were maintained in a safe, operable condition.

Evidence:
1. Observed in the school age classroom a blue book case that had exposed screws on the bottom of the book shelf. The book shelf was also unstable and posed a entrapment hazard to the children.

Plan of Correction: The bookcase will be discarded and will be monitored.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that all chemicals were locked.

Evidence:
1. In the art area of the school age room there was a closet that had 12 containers of cleaning chemicals that were unlocked and accessible to the children.

Plan of Correction: The closet door has a sign on the door to keep it locked at all times. The director will remind staff to check it in the mornings since this is a shared space.

Description: Based on observation of the two year old room, the center failed to ensure that all topcals were used in accordance with the manufacturer?s recommendation.

Evidence:
1. There was a container of sunscreen for child # 6 that expired 1/22.

Plan of Correction: The sunscreen was discarded and all staff will monitor dates on sunscreens as they are brought in.

Standard #: 8VAC20-780-530-A
Description: Based on review of five staff?s records, the center failed to ensure that each classroom had a staff person with First Aid and CPR certification.

Evidence:
1.Staff # 4 was hired on 8/5/2022 and was working alone in the 3 year old and did not have First Aid and CPR certification.

Plan of Correction: First Aid and CPR will be scheduled ASAP and will be scheduled for all staff as soon as they are hired.
