Comments:
An unannounced monitoring inspection was conducted with the director on October 13, 2022 beginning at 11:32 AM and ending at 1:03 PM. There were 21 children, ages 3 through 5 years old, in the direct care of 6 staff members. The children were observed playing with age appropriate toys and games and being read a book. One class was observed being dismissed to their parents. Five children's records were reviewed and in compliance. Three staff records were reviewed. There were no medications on site to be inspected. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent evacuation drill was documented as 10/11/2022. The most recent shelter-in-place drill was documented as 9/20/2022. Areas of non-compliance are identified in the Violation Notice.

Description: Based on observation, the facility staff did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevents access by children. Evidence: In the 4?s classroom, there was an unlocked cabinet that had 6 boxes of borax.

Plan of Correction: The unlocked closet was designated for use by the school aged program. The preschool program staff was unaware that it had been left unlocked. The director spoke with the preschool staff to ensure that all the closets in the classroom were locked. The director also stated that she would follow up with the school aged program staff regarding securing their closet after use.
