Comments:
An unannounced non-mandated monitoring inspection was conducted on-site November 22, 2022 as a follow-up to an intensive plan of correction regarding hazardous substances and background checks. The director was available during the inspection. There were 61 children present with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, and special care and emergencies. A total of 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation and interview on November 22, 2022, the center failed to ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence: 1. The Big Room contained 3 bottles of Gillette shaving cream in an unlocked cabinet. The labels on the shaving cream contained the language "keep out of reach of children" and "caution." 2. Staff #3 stated they shouldn't have been in an unlocked cabinet.

Plan of Correction: Hazardous substance (Shaving cream) was immediately removed from the individual container where it was stored in the wall cabinet 6.5 feet off the ground.

Standard #: 8VAC20-780-500-B
Description: Based on observation and interview on November 22, 2022, the center failed to ensure a nonabsorbent surface for diapering or changing shall be used. For children younger than three years, this surface shall be a changing table or countertop designated for changing.
Evidence: 1. Staff #1 in the toddler room (ages 2 and 3) was observed during diapering. 2. Staff #1 was observed sitting in a chair in the bathroom. The children were seated on a bench until their turn. 3. Child #1 (age 2) was observed to stand in front of staff #1. Staff #1 then removed the pull-up, wiped child #1, and then put a dry pull-up on child #1, all while standing. 4. Staff #3 stated that the center has always done wet diaper changes that way.

Plan of Correction: An allowable variance has been applied for by the director.
