Provided consultation on standard:
1. 160A2: Each staff member shall submit documentation of a negative tuberculosis (TB) test/screening that has been completed within the last 30 calendar days of the date of employment.
2. 80: For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. The attendance record helps first responders, and others in an emergency, easily identify who is and is not in the classroom at any given time.
3. 240I: documentation of orientation training shall be kept by the center in a manner that allows for identification by the individual staff member and shall include:
a. Name of staff
b. Training topics
c. Training delivery method
d. The entity or individual providing training
e. The date of the training
4. 560J: tables shall be sanitized before and after each use for feeding; if children are drinking milk at a table (ie such as a bottle in the toddler class) tables must be sanitized (and hands washed before/after)

Comments:
An unannounced monitoring inspection was conducted from 9:45 am - 12:45 pm with the center director. There were 64 children, ranging from three months to five years of age, present with 12 staff supervising. Children were observed engaged in music time, arts and crafts, table toys, playing on the playground and in the gym. Five child records and five staff records were reviewed, along with the emergency drill log, written daily attendance, written allergy list, and the fire and health inspections.

Violations:
Standard #: 8VAC20-780-40-L
Description: Based on staff interview, the facility failed to inform all staff who work with children of children's allergies, sensitivities and dietary restrictions. Evidence: when asked, not all staff could state which children in the group had allergies, sensitivities and/or dietary restrictions.

Plan of Correction: The Goddard of Manassas has informed staff of children allergies, sensitivities, and dietary restrictions. Each classroom includes a clipboard that is accessible to all faculty members and kept confidential using a cover sheet. Reports are updated monthly and as new children with allergies, sensitivities, and dietary restrictions enroll. As reports are updated, administration informs faculty of
updated allergies, sensitivities, and dietary restrictions. In addition to classroom clipboards, the Goddard School of Manassas has included a second form of communicating to faculty children?s allergies, sensitivities, and dietary restrictions using our communication platforms; teachers are able to see children?s profiles that include specific allergies, sensitivities, and dietary restrictions for the school and for their individual classrooms. In addition, Administration visits each classroom every fifteen minutes. Faculty is asked to state the names of children in their classroom with allergies, sensitivities, and dietary restrictions upon each visit.

Description: Based on review of documentation, and an interview with the director, the facility failed to maintain in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present. Evidence: The list posted in the classrooms, dated: 11/11/22, did not include child # 6, with food allergies and sensitivities. Staff stated that the list did not accompany children to the gym or playground.

Standard #: 8VAC20-780-70
Description: Based on review of five staff records, and an interview with the center director, it was determined that not all records contained all required information. Evidence: The records for staff #1, #2, #3 and #4 did not contain complete documentation of two reference checks as required. Verbal references taken over the phone did not have dates or signatures of the person making the call.

Plan of Correction: Upon interviewing candidates, candidates are required to provide a minimum of two professional references to contact. When a candidate is hired at the Goddard school of Manassas reference checks have already been obtained prior to hire. References obtained include the following: character and reputation as well as competency, dates of contact, names of persons contacted, firms contacted, results of reference questions and signature of person making call. Administration at The Goddard School of Manassas has adopted a detailed checklist that serves as a follow-up check for a second administrator to have the chance to review completion of all forms including a verbal reference which includes the following: dates of contact, names of persons contacted, firms contacted, results of reference questions, and signature of person conducting the reference.

Standard #: 8VAC20-780-500-A
Description: Based on observation, the facility failed to ensure that children wash their hands with soap and water or disposable wipes before and after eating. Evidence: An infant was fed a bottle without having their hands washed beforehand and a toddler did not wash their hands after drinking a bottle (of milk) in the toddler class.

Plan of Correction: The Goddard school of Manassas has held a faculty meeting with all faculty covering the following topics: best handwashing practices, prevention of communicable diseases. The Goddard school department of quality assurance has provided mandatory retraining for all faculty in efforts to ensure proper handwashing for all faculty and children before and after meals. Administration
visits each classroom every fifteen minutes, faculty and children are observed for proper handwashing before and after meals.

Standard #: 8VAC20-780-500-B
Description: Based on observation, and an interview with staff, the facility failed to ensure that diapers are disposed of in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: The foot pedal operated trash can lid in the toddler class is broken, and has been inoperable since approximately 5/22. Diapers were observed inside the can.

Plan of Correction: Succeeding the toddler classroom observation, the facility provided the classroom with a trash can that ensures diapers are disposed of in a leak or plastic lined storage system that includes an operating foot peddle. Neither the staff member?s hand nor the soiled diaper touches an exterior surface of the storage system during disposal. The classrooms that have diaper changing tables at The Goddard of Manassas have operational foot petals or plastic lined storage system for the children?s diapers. The faculty in the classrooms that have diaper changing tables are aware that if the storage system is not in working condition to contact administration for a proper working storage system. In addition, Administration conducts monthly Quality Assurance observations. Quality Assurance includes to inspect that the storage system is working properly and are replaced if necessary.

Standard #: 8VAC20-780-560-M
Description: Based on observation, the facility failed to ensure that no child is allowed to drink while walking around. Evidence: The Licensing inspector observed several children in the toddler class walking around drinking from their water bottles.

Plan of Correction: The Goddard School of Manassas provides faculty with trainings upon hire that encourages children to drink from their water bottles throughout the day. Faculty has been trained to encourage children to take water breaks throughout the day, the Goddard school of Manassas has retrained all faculty on proper procedures when children are drinking from their water bottles and water cups. Administration visits each classroom every fifteen minutes; during classroom visits, administration is monitoring children who drinking from their water bottles or water cups and ensuring that all children are seated while drinking.
