Comments:
An unannounced monitoring inspection was initiated on 12/6/2022 and concluded on 12/7/2022. The inspector was on site on 12/6/2022 from approximately 9:20 am-11:25 am. There were 23 children present, ranging in ages from 3 to 5 years, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, nutrition and background checks. A total of 5 child records and 5 staff records were reviewed. Staff records were reviewed remotely on 12/7/2022.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence:
1. Areas of rust were observed on the top of the chain linked fence on the preschool playground. Two areas of rust were observed in the bathrooms of the Sesame Street and Sugartown classrooms.
2. Administration acknowledged that the areas contained rust.

Plan of Correction: Of the approximately 4.500 sq feet of indoor space which was inspected, two small areas of rust were located. Each was 3 1/4 inch in diameter, on a piece of toilet plumbing. Each piece was located behind a toilet, accessible to children but not very easily. Both of these cosmetic pieces were removed the day of inspection. The rust that was observed on the top rail of the chain link fence will be covered with anti rust paint this week.

Description: Based on observation, review of documentation and interview, the center did not ensure that at least one staff member in each classroom or area where children are present had current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care.

Evidence:
1. Staff #1 was observed independently supervising a group of four (4) children in the Disney room. (four year old children) The record of staff #1 did not contain documentation of a CPR certification.
2. Administration acknowledged that they did not have record of the certification.

Plan of Correction: Staff #1 was not "certified" but was "trained" in standard first aid and CPR which follows Federal Law but not Virginia State Code. There was however another staff who met all "Virginia" certification requirements in the classroom directly across the hall, approximately 20 feet away. Staff #1 will be "certified" this week.
