An unannounced complaint investigation was initiated on 11/17/22 with two local agencies in response to a complaint received regarding allegations of child abuse and lack of supervision. The investigation was continued on 11/18/22 with documentation and records review. Based on a review of video footage and interviews conducted, the allegations were determined to be not valid. There were violations cited on the Violation Notice that were not reported in the complaint but were found during the investigation. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 12/12/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-780-70 Complaint related: No Description: Based on review of two staff records, the center did not obtain all of the required documentation for staff records. Evidence: The record for Staff #2 did not contain documentation to demonstrate that the individual possessed the education required by the job position. Plan of Correction: All required documentation has been placed in staff files.
Standard #: 8VAC20-780-240-A Complaint related: No Description: Based on review of two staff records, the center did not ensure that all staff completed the Virginia Department of Education (VDOE)-sponsored orientation course within 90 calendar days of employment. Evidence: The record for Staff #2, with an employment date of 3/3/22, did not contain documentation that the staff had completed the VDOE-sponsored orientation course. Plan of Correction: All staff will complete the required course within the proper timeframe moving forward.
