An unannounced monitoring inspection was conducted on 12/14/2022 from 10:00 a.m. to 11:45 a.m. There were 33 children present, ranging in ages from 2 years old to 5 years old, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 6 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A Description: Based on review of records, the center failed to ensure that each child had a physical examination prior to or within 30 days of the first day of attendance,
Evidence:
1. A physical examination was not in Child #2?s record. Child #2?s start date was 9/26/2022.
2. Staff #1 confirmed that there was not a physical examination in Child #2?s record.Plan of Correction: Will call child's mom to get a copy of the physical to add to shot record.
