Attendance records; reports. For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person tour of the program. A monitoring inspection was initiated on December 19, 2022 and concluded on December 20, 2022. There were 47 children present, ranging in ages from 5 to 11 years old with 3 staff supervising. The inspector reviewed compliance in the areas of administration and staffing and supervision only this was a focused inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-350-B-5 Description: Based on observation, interviews and a review of documentation the licensee did not ensure that the required child to staff ratio was met. The required ratio for school age children from 5 years to 9 years old is 1 staff for every 18 children.
Evidence: On December 19, 2022 the license failed to meet the required child to staff ratio is listed below;
1) At approximately 3:31 pm, 47 children arrived at the after-school program and there were only 2 staff members (Staff #1 and Staff #2) onsite providing supervision.
2) At 3:43 pm another staff member (Staff #3) arrived on site and then immediately Staff #2 left the center. This was confirmed by Staff #3 and Staff #1.
3) At approximately 3:57 pm a third staff member, Staff #4 arrived onsite and now there were approximately 47 children with 3 staff. This was confirmed by Staff #4.
4) From approximately 3:31 pm until 3:57 pm there were only 2 staff members with 47 children
5)The age range of children in care was 5 to 11. Staff #1 stated that 2 staff had called out and they were short staffed.Plan of Correction: Program manger will notify supervisors as soon as call outs occur so that timely coverage can be provided for ratio. Any staff sent to cover will be required to stay to maintain ratio until relieving staff have arrived.
