Licensing Inspector conducted an unannounced Monitor Inspection on December 12, 2022. There were 10 children ages 4 to 7 years old present under the direct care of 2 staff. The Licensing Inspector reviewed 3 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-A Description: Based on record review & discussion with staff, the Licensee failed to ensure each staff member submit documentation of a negative tuberculosis screening.
EVIDENCE:
Licensing Inspector reviewed 2 staff records and attempted to review a third staff record. Record for staff 2 was unavailable. Staff 2 was observed working alone with and in control of the children in care. TB skin test/screening was unavailable.Plan of Correction: Staff will schedule an appointment with health official to complete TB skin test or TB screening. Documentation of the findings will be maintained in the staff record.

Annual health inspection has not been complete. Most recent inspection last completed 8/24/2021Plan of Correction: Health official has a scheduled appointment to complete annual health inspection. Documentation of the findings will be maintained on file.
Standard #: 8VAC20-780-270-C Description: Based on inspection, the Licensee failed to keep inside temperature in areas occupied by children maintained no lower than 68 degrees F.
EVIDENCE:
Inside temperature registered at 61 degrees F. Temperature was not maintained at a minimum of 68 degrees F.Plan of Correction: Corrected during inspection. Temperature was increased to 68 degrees F.
Standard #: 8VAC20-780-320-B Description: Based on inspection, the Licensee failed to ensure hot water temperature at taps used by the children in car, do not exceed 120 degree F.
EVIDENCE:
Hot was temperature registered as 124 degrees F. Hot water temperature exceeded maximum temperature of 120 degrees F.Plan of Correction: Staff will contact building maintenance to adjust hot water temperature.
Standard #: 8VAC20-780-540-E Description: Based on inspection, the Licensee failed to have required emergency supplies.
