Violations:
Standard #: 8VAC20-780-240-A
Description: Based on review, 2 of 13 staff files indicated that not all new staff obtained the Virginia Department of Education-sponsored orientation course (Virginia Pre-Service 10 hour training) within 90 calendar days of their employment.

Evidence -
1. On the date of inspection (01/06/2022) a certification of completion for the 10 hour Virginia Pre-Service training was not available for review for: Staff #4 and Staff #5.
2. Their start date was listed as 05/31/2022 for both Staff #4 and Staff #5.
3. The Director stated (during an interview conducted on the date of inspection) and provided written documentation that both Staff #4 and Staff #5 were reminded in October and again in December of 2022 about the need to complete this course.

Plan of Correction: This course will be completed.

Standard #: 8VAC20-780-330-A
Description: Based on observation, a possible hazard was observed on the Pre-K playground.

Evidence -
1. On the date of inspection (01/06/2023), a tripping hazard was observed on the Pre-K playground. The white cloth layer from underneath the resilient surfacing was observed protruding through the mulch.
2. This exposed flap of cloth may serve as a possible tripping hazard if not addressed.

Plan of Correction: The playground will be re-raked. Additional surfacing may be ordered if needed.

Standard #: 8VAC20-780-550-F
Description: Based on review, the lockdown procedures have not been practiced on an annual basis.

Evidence -
1. After a review of the drill log it was determined that a lockdown drill has not been conducted since October 13, 2021 when this standard went into effect.
2. The Director confirmed this finding.

Plan of Correction: We will need to asses our options and consult with the fire marshal's office for their approval. Once those things are in place, the center will move forward in the practice of lockdown procedures.
