Technical Assistance:
1. Discussed employee reference check requirements.
2. Reviewed cot spacing requirements during nap times.
3. At least one staff person in each room should sit with the children while they eat.

Comments:
Conducted an unannounced inspection at 11:05am. Observed 77 children + 12 direct-care staff. Ratios and supervision were in compliance. Children were having outdoor active play, eating lunch, washing hands, and taking naps. Staff were promptly tending to the needs of the children. The facility was found to be clean and well supplied with toys and equipment. Recordkeeping was in order. Areas of non-compliance were found with standards and laws reviewed. Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observations made, it was determined that a piece of playground equipment was not maintained in a safe condition. Evidence: One side of the see-saw structure was not attached to the spring base. The inspector and Director were able to lift the see-saw main beam from the spring base. When lifted a space between the beam and spring was created that presented as a potential pinch point.

Plan of Correction: The Director stated that he would have the equipment repaired.

Standard #: 8VAC20-780-440-J
Description: Based on observations made, it was determined that occupied cribs did not have a minimum of 30? of space on the service side. Evidence: A total of 5 occupied cribs in the infant room had approximately 12?-18? of space on the service side.

Plan of Correction: Cribs were moved during the inspection to create the required space. The Director stated that he would review this with staff after the inspection.

Standard #: 8VAC20-780-570-E
Description: Based on observations made, it was determined that infant bottles were not labeled with the date. Evidence: A total of 3 infant bottles were not labeled with the date.

Plan of Correction: The Director stated that he would remind parents to label bottles daily.
