Comments:
An unannounced monitoring inspection was conducted on Thursday, December 29, 2022 from 10:10am to approximately 12:45pm. There were a total of 18 children in care in the direct care of 10 staff members. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility including classrooms, hallways, playground, and bathrooms were inspected. The required postings were reviewed and found to be in compliance. Medication is administered and medications and authorizations were reviewed. The center is equipped with toys and supplies and items were available to the children. Five children's records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of five staff records and interview, the center did not ensure one staff member had documentation of a negative tuberculosis (TB) screening within the required
timeframe.

Evidence: 1) The TB screening in the record of Staff #3, employed on 06/20/22, was dated 03/11/22. The documentation shall have been completed within the last 30 calendar days of
the date of employment. 2) During interview, management confirmed the documentation was not received within the required time frame.

Plan of Correction: Moving forward, management will make sure TB screening/testing are dated within 30 days of employment.

Standard #: 8VAC20-780-70
Description: Based on a review of five staff records and interview, the center did not ensure one staff record contained the required information.

Evidence: 1) The record of Staff #2, employed on 09/28/22, did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering. 2) During interview, management acknowledged the record of Staff #2 did not have any references on file.

Plan of Correction: Two references were obtained on 12/29/22. In the future, staff will have two references prior to employment.

Standard #: 8VAC20-780-510-F
Description: Based on a review of four medications and interview, the center did not ensure that three medication authorizations were available to staff during the entire time they were effective for three out of four children whose medications were reviewed.

Evidence: 1) A prescription medication was observed for Child #6 and no written parental authorization was present for this medication. 2) A prescription medication was observed for Child #7 and no written parental authorization was present for this medication. 3) A prescription medication was observed for Child #9 and no written parental authorization was present for this medication. 4) During interview, management the parental authorizations for these medications were not present.

Plan of Correction: The missing written authorizations will be obtained by the end of the week of 01/04/23. Moving forward, management will ensure parental and physician signatures are included for all long-term medications.
