Comments:
The inspector conducted an unannounced monitoring inspection on Tuesday, January, 3, 2023. The inspection was conducted at approximately 9:15 AM until 11:15 AM. There were 61 children in care with 12 staff supervising. The inspector observed the children playing with various age appropriate play materials, napping, and playing with table activities. The inspector inspected all 6 classrooms, restrooms, playground, kitchen, and one bus. The center's first aid kit and non-medical emergency supplies were inspected and found complete. Liability insurance, health inspection, and drills were all in compliance. A total of 5 child records, and 5 staff records were reviewed. Violations were cited during this inspection. Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and inspection of the facility, the licensee did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.
Evidence:
1. An area of the rug in the school age classroom near the back door was torn and the material was lifted up causing a potential tripping hazard for children.
2. There are two cots with torn and frayed areas in the 2's classroom.
3. One outlet cover on a lower area of wall in the 2?s classroom was broken off at the top resulting in exposure of the electrical unit within the wall.

Plan of Correction: 1. The rug will be removed today.
2. Cots will be replaced this afternoon.
3. Will be covered within an hour.

Description: Based on review of medications, the center did not have written authorization to administer medication to 3 children in care.
Evidence:
1. Child # 6 had an emergency rectal medication on-site, without written authorization from the parent and physician.
2. Child # 7 had an Epi-pen medication on site, without a written authorization from the parent and physician.
3. Child # 8 had an Epi-Pen medication on site, without a written authorization from the parent and physician.

Plan of Correction: Forms were faxed over to the doctors awaiting completion.
