Comments:
An unannounced renewal inspection was conducted today between the hours of 9:45am- 1:45 pm. There were a total of 259 children with 67 staff members present. Children were observed during circle time, morning snack time, individual table activities, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 8VAC20-780-160-A
Description: Based on record review, documentation for a tuberculosis screening was not completed within the last 30 calendar days of the date of employment.
Evidence:
1. Staff #3's record (Date of hire-12/06/2022) contained documentation of a tuberculosis screening dated 10/25/2022. This tuberculosis screening results were not completed within the last 30 calendar days of the date of employment.

Plan of Correction: This staff member?s start date was delayed causing the time between the test and start date to be just over 30 days. We will request additional information if a situation like this happens again in the future.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the K4 classroom (Room 1), a bottle of electronic wipes with the label, "Keep Out of Reach of Children", was located in an unlocked low cabinet.

Plan of Correction: The wipes were immediately removed from the classroom and the safety lock was repaired.

Description: Based on observation, cosmetics, medications, or other harmful agents were not stored in areas that were not accessible to children.
Evidence:
1. In the K4 classroom(Room12), a container of Vaseline was located in an unlocked low drawer.

Plan of Correction: The container was immediately removed from the room and a safety lock was added to the drawers and the teachers rechecked all the drawers.

Description: Based on observation, cots were not identified for use by a specific child.
Evidence:
1. In the K2 classroom (Room44), the cots were not identified for use by a specific child.

Plan of Correction: Permanent numbers were marked on the cot for a cot chart to replace tape that continued to be peeled off.

Standard #: 8VAC20-780-510-H
Description: Based on observation and interview, a nonprescription medication was not in the original container with the direction label attached.
Evidence:
1.Child #8 has a diagnosed food allergy and has emergency medication on site. A bottle of emergency medication(Diphenhydramine) was located in a locked medication sac without the original container with the direction label attached. The epinephrine pen was also located in the locked medication sac.
2. Staff #11 confirmed that the original container was missing from the medication sac.

Plan of Correction: The medication was removed from the class and the parents were contacted for the correct items. All medication check-in procedures were reviewed with the appropriate staff.
