22.1 BACKGROUND CHECKS, CODE, CARBON MONOXIDE

Comments:
An announced initial inspection was conducted on 01/18/2023. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, special care provisions and emergencies, and background checks. A total of 1 applicant record was reviewed. The center was measured to determine the license capacity and the required emergency supplies and policy/procedures were reviewed. The inspector arrived for the inspection at 9:45 am and departed the center at 1:15 pm.

Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-40-K
Description: Based on document review, the center failed to develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.

Evidence: The center?s shaken baby syndrome procedures did not include coping with crying babies or information on sudden infant death syndrome awareness.

Plan of Correction: The procedures will be updated to include all of the requirements.

Standard #: 8VAC20-780-40-N
Description: Based on document review the center failed to develop written playground safety procedures that shall include all of the elements as required by the standards.

Evidence: The center?s playground safety procedures did not include provisions for active supervision by staff to include positioning of staff in strategic locations, scanning play activities, and circulating among children.

Plan of Correction: The procedures will be updated to include all of the requirements.

Standard #: 8VAC20-780-550-B
Description: Based on document review, the center failed to ensure that the emergency preparedness plan shall contain all of the procedural components as required by the standards.

Evidence:
1. The emergency preparedness plan did not contain the following information: sounding of alarms, such as intruder, evacuation, lockdown and shelter-in-place for tornado or chemical hazards; the establishment of center emergency officer and back-up officer to include 24 hour contact telephone number for each; notification of local authorities, such as fire and rescue, law enforcement, emergency medical services, poison control, health department, and parents and local media; and the availability and primary use of communication tools.
2. The evacuation procedures did not include the following information: primary and secondary means of egress and complete evacuation of the building; methods to ensure any health care needs to include medications and care plans, emergency contact information for staff and supplies are taken to the assembly point or relocation site; and accommodations or special instructions for toddlers and children with special needs to ensure their safety during evacuation or relocation.
3. The shelter-in-place procedure did not include the following information: scenarios applicability, such as tornado or chemical spill, inside assembly points, methods to account for all children at the safe locations, and primary and secondary means of access and egress; methods to ensure any health care needs to include medications and care plans, emergency contact information for staff and supplies are taken to the assembly point or relocation site securing of essential documents, including attendance records, parent contact information for staff and supplies are taken to the assembly point or relocation site; accommodations or special instructions for infants, toddlers and children with special needs to ensure their safety during evacuation or relocation; and procedures to reunite children with a parent or authorized person designated by the parent to pick up the child.
4. The lockdown procedures did not include the following information: accommodations or special instructions for infants, toddlers and children with special needs to ensure their safety during evacuation or relocation.
5. The emergency preparedness plans did not include information regarding the continuity of operations to ensure that essential functions are maintained during an emergency.

Plan of Correction: The procedures will be updated to include all of the requirements. Will use the model form.
