Comments:

A complaint investigation was begun on 1/3/2023 in response to an allegation being received in our office. Based on multiple staff interviews and video footage, the preponderance of evidence supports that allegation of a lack of supervision. The complaint is Valid. Description:	Based on a review of records, one staff record does not contain documentation of lead teacher qualifications. Evidence: Documentation submitted for review does not total 24 hours of training within the allowable 30-day period (the requirement at that time, 2017). Documents provided total 16.08 hours. (Staff A)
Plan of Correction:	Staff documentation will distinguish staff as qualified as program leader to include 24 hours of training within 30 day period.

Description:	Based on a review of video footage the facility did not ensure care and protection for a 4-year-old child in care. Evidence: On 1/3/2023 a 4-year-old child arrived at a program at another facility with 3 marbles in his mouth. It was determined through reviewing video footage that the child had placed the marbles in their mouth at the daycare at 8:18AM and had them in the mouth for approximately 17 minutes at the time they left the center at 8:35AM. (Child 1: DOB 12:14/18)
Plan of Correction:	Staff was immediately retrained on 1/3/23 to ensure age-appropriate materials in classroom & provide care and protection. Facility documented corrective action of staff.

Description:	Based on a review of footage, a child under the age of 10 was not kept within actual sight and sound supervision. Evidence: On 1/3/23 a 4- year-old child was not in sight and sound supervision. Video shows the staff member seated at a table while a child is across the room with his back to the teacher, playing with marbles, behind a shelf unit.
Plan of Correction:	Staff was immediately retrained on 1/3/23 to maintain sight and sound supervision by movement of staff. Furniture was immediately rearranged to provide supervision.
