An unannounced monitoring inspection was conducted on January 27, 2023 from approximately 11:00 am to 12:30 pm. There were 22 children in attendance and a total of 7 staff present. 2 staff records and 3 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the kitchen. The children were observed playing with blocks, watching a video, and eating lunch while the licensing inspector was on-site.
The most recent health inspection was on 10/05/22.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 01/31/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #: 8VAC20-780-160-A Description: Based on record review, the center failed to ensure that each staff member submits documentation of a negative tuberculosis screening no more than 30 calendar days prior to the date of employment. Evidence: The record for staff #2, date of employment 10/11/22, contained documentation of tuberculosis screening dated 8/20/21. Plan of Correction: Not available online. Standard #: 8VAC20-780-550-D Description: Based on documentation review, and interview, the center failed to document monthly practice evacuation drills. Evidence: Review of the evacuation drill documentation revealed no drills were documented for the months of August, September, October, November, or December of 2022.
