Comments:
An unannounced monitoring inspection was conducted on 12/21/22 at 8:50am to 10:10am with another licensing inspector and continued on 1/3/22 at 10:00am to 11:25am with the local agency and licensing inspector to investigate a self-reported incident at the center regarding the sight and sound supervision and the care and protection of children in the Two-year-old classroom where a child was left alone while the class went outside for their playground time. Interviews were conducted with 4 staff members. Attendance documentation, 1 child file, and 4 staff files were reviewed. The Two-year-old classroom was also observed during inspection as well as a video containing footage of the incident. An exit interview was conducted on 1/31/2023. Violations:
Standard #: 63.2-1509-A
Description: Based on staff interview and documentation, the center failed to report a suspected incident of child abuse or neglect to Child Protective Services within 24 hours as required.

Evidence:
On 12/16/2022 at approximately 10:00am, Child #1 (DOB 11/18/20) was left in a classroom unattended for approximately 27 minutes. Based on documentation, the center reported the incident to Child Protective Services on 12/21/22 at approximately 5:00pm.

Plan of Correction: Director notified the licensing representative immediately regarding the incident. CPS was contacted once the licensing representative advised the director. Going forward Child Protective Services will be contacted as required.

Standard #: 8VAC20-770-60-B
Description: Based on review of 4 staff records, the center did not obtain documentation of a completed sworn disclosure statement for each staff prior to date of hire.

Evidence: The record for Staff #2 (date of hire 8/1/22) did not contain documentation of a completed sworn disclosure statement prior to the date of hire. The sworn disclosure on file was dated 9/16/22.

Plan of Correction: All staff will complete the sworn disclosure statement by the date of hire.

Standard #: 8VAC20-780-340-A
Description: Based on staff interviews and review of video footage, the center did not ensure the care, protection, and guidance of all children in care.

Evidence:
1)On 12/16/22 at approximately 10:00am, Child #1 (DOB 11/18/20) was left alone in the classroom while the rest of the class went out to the playground.
2)The video footage showed that the child began crying approximately 3 minutes after the group left the classroom and cried at the door while wearing a heavy coat with a hood for the remaining 24 minutes the group was outside on the playground.
3)The staff stated the child was visibly upset, had a red face and looked afraid when they returned to the classroom.

Plan of Correction: To ensure proper supervision, care, protection, and guidance of all children in care, our face to name transition procedures were reviewed with all staff members the following day on December 17, 2022.
On January 5, 2023, during our staff meeting, all staff were formally re-trained on all face to name procedures to include role modeling, and proper use of transition tools.
On January 10th and 12th, a follow-up training occurred with individual classes.

Standard #: 8VAC20-780-340-F
Description: Based on staff interviews and review of video footage, the center did not ensure that children under 10 years of age always be within actual sight and sound supervision of staff.

Evidence:
1)On 12/16/22 at approximately 10:00am, Child #1 (DOB 11/18/20) was left alone in the classroom while the rest of their class went out to the playground.
2)The video footage of the Two-year-old classroom showed that Child #1 was alone for 27 minutes while the Two-year-old group was outside on the playground.
3)Staff #1 and Staff #2 stated that they did not realize they had left the child inside until it was time to return to the classroom with the rest of the group.

Plan of Correction: To ensure proper supervision, care, protection, and guidance of all children in our care, our face to name transition procedures were reviewed with all staff members the following day on December 17, 2022.
