Comments:
An unannounced monitoring inspection was conducted on January 31, 2023 from 9:30 am until 11:30 am. There were 20 children present with 5 staff supervising. The physical plant, children's records, staff records, attendance logs, evacuation logs, menu's, cots and emergency procedures were reviewed. There were no medications on site. The children were observed listening to a story. Areas of non-compliance were cited on the Violation Notice. Violations:
Standard #: 22.1-289.035-B-2
Description: Based on a review of staff records, the center failed to ensure that staff have Criminal History records checked prior to the first day of employment.

Evidence:
Staff #1 and #3's records do not contain documentation of a Criminal History background check by the first day of employment.

Plan of Correction: All staff will be required to have criminal history background checks completed before the first day of employment.

Standard #: 8VAC20-780-130-A
Description: Based on a review of children's records, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
Child #4's record does not contain documentation of immunizations by the first day of attendance. First day of attendance was 09/06/2022, the documented date of immunizations was 10/10/2022.

Plan of Correction: All newly enrolled students will be required to present documentation of immunizations on the first day of attenddance.

Standard #: 8VAC20-780-60-A
Description: Based on a review of children's records, the center failed to obtain all required documentation for each child enrolled.

Evidence:
Child #5's record did not contain documentation of a phone number for the child's physician.
Child #3's record did not contain documentation of the names of persons authorized to pick up the child/

Plan of Correction: I will contact parents and obtain required documentation for each child.

Standard #: 8VAC20-780-230-B
Description: Based on a review of staff records, the center failed to ensure that staff shall complete orientation training in subsection C of this section prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

Evidence:
Staff #1 and #2's records do not contain documentation of orientation by the 7th day of employment.

Plan of Correction: All new staff will received orientation training by the 7th day of employment and it will be documented in each staff member's file.

Standard #: 8VAC20-780-240-C
Description: Based on a review of staff records, the center failed to ensure that all staff complete the training "Recognizing Child Abuse and Neglect" and the legal requirements for reporting suspected child abuse as required by 63.2-1509 of the Code of Virginia.

Evidence:
Staff #3's record does not contain documentation of the completion of Recognizing Child Abuse and Neglect training.

Plan of Correction: All new staff will be required to complete the Recognizing Child Abuse and Neglect training and provide documentation that will be placed in their file.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In the girl's bathroom, there was a bottle of household cleaning on the shelf above the changing table and there was a can of Lysol on a shelf in the bathroom.

Plan of Correction: The hazardous materials were locked away during the time of the inspection and I will remind staff that all hazardous substances must kept in a locked place.
