An unannounced renewal inspection was initiated on 1/9/2023 onsite from 3:15 pm to 4:30 pm. Fifteen children, ages 6 through 9 years of age, were in one group under the care of three staff. The Licensing Inspector reviewed three children?s records. The children were observed in free play, washing hands, having a snack, and preparing for more play in another area.
On 1/17/2023 the inspection was concluded at the main office from 3:10 pm to 4:05 pm. Three records were reviewed, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the center failed to have emergency contact information for staff onsite at the center.
EVIDENCE:
The records did not contain emergency contact information for staff 1, 2, and 3 who were onsite on the day of the inspection. Staff 1 stated they weren?t at the site.Plan of Correction: Not available online. Contact Inspector for more information.
