Standard #: 8VAC20-780-60-A-8 Description: Based on record review and staff interview, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of suspected or confirmed allergic reaction.
Evidence: Child #6 and Child #7 both have diagnosed food allergies and neither child had a written allergy care plan on file signed by a doctor.Plan of Correction: We have filled the appropriate forms out and are just waiting on doctor's signature.
Standard #: 8VAC20-780-240-A Description: Based on record review, the center did not ensure that all staff complete the Virginia Department of Education sponsored orientation course within 90 calendar days of employment.
Evidence: Staff #1 (Date of Hire: 9/1996) did not have documentation on file of having completed the Virginia Department of Education sponsored orientation course.Plan of Correction: This course will be completed.
