Comments:
An unannounced renewal inspection was conducted on Wednesday, February 1, 2023. The inspector was on site from 10:00am to approximately 1:05pm. There were a total of 76 children in care in the direct care of 13 staff members. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility used by the children were inspected. The center is equipped with toys and supplies and items were available to the children. The required postings were reviewed and found to be in compliance. Medication is administered when required and medications were reviewed. During the inspection, seven children's records, seven staff records, and three officers' background checks were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: Based on a review of seven staff records and interview, the center did not obtain a required background check for one staff member every five years.

Evidence: 1) The most recent sworn statement in the record of Staff #7 was dated 08/01/2017. A repeat sworn statement should have been completed no later than 08/01/2022.

2) A member of management confirmed an updated sworn statement could not be located for Staff #7.

Plan of Correction: Staff # completed a new sworn statement on 02/01/23. Moving forward, documentation of updated background checks will be maintained for each staff member.

Standard #: 8VAC20-780-280-B
Description: Based on observations, the center did not ensure to keep hazardous substances such as cleaning materials in a locked place using a safe locking method that prevents access by children.

Evidence: 1) A bottle of disinfectant spray was observed in an unlocked cabinet in the Honeycrisp classroom.

2) Two bottles of cleaner, one can of air freshener, and a container of disinfectant wipes were observed in unlocked cabinets in two of the bathrooms used by children in the Gala classroom.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Plan of Correction: The cleaning supplies will be moved to cabinets that have a safe locking method to prevent access by children.

Standard #: 8VAC20-780-550-F
Description: Based on a review of records and interview, the center did not practice the center's lockdown procedures at least annually.

Evidence: Evidence: 1) The licensing inspector observed that the most recent emergency response drill log. The last lockdown drill was documented in 2021.

2) A member of management confirmed a lockdown drill was not conducted in 2022. The lockdown procedures should be practiced at least annually.

Plan of Correction: Moving forward, the lockdown procedures will be practiced and documented annually.

Standard #: 8VAC20-780-550-H
Description: Based on a review of records and interview, the center did not maintain a record of the dates of the practice drills for one year.

Evidence: 1) The licensing inspector observed that the most recent emergency response drill log. There was no record of a second shelter-in-place drill in 2022. Shelter-in-place procedures shall be practiced a minimum of twice per year.

2) A member of management confirmed the second shelter-in-place drill was conducted on 11/11/2022, but it was not documented as required.

Plan of Correction: Moving forward, the required documentation will be maintained for each emergency response drill conducted.
