Comments:
A monitoring inspection was conducted on 1/25/2023 with the center director and staff. There were 54 children present, ranging in ages from 8 months to 5 years, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of 5 staff records, the center failed to obtain documentation of a negative tuberculosis (TB) screening or test for each staff at the time of employment and prior to coming in contact with children. Evidence: Staff 1 (date of employment 1/03/2023) did not have a TB test until 1/23/2023.

Plan of Correction: All staff will provide a TB test or screening prior to direct contact with children.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the facility did not maintain all areas of the center in a clean, safe, and operable condition. In the preschool room, there was peeling paint and drywall, a couple of inches in diameter, on the corner of the wall near the exit. The hole was close to the floor and was accessible to the children in care. In the school age room, there was a hole on the drywall near the far exit door, accessible to children.

Plan of Correction: The walls will be patched and painted by a contractor.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the facility failed to keep hazardous substances in a locked place that prevents access by children. Evidence: In the twos classroom, the cleaning solution and sanitizing solution were in an unlocked upper cabinet.

Plan of Correction: Locks were purchased for the cabinets that contain hazardous substances.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the facility failed to use the diapering surface for diapering only. Evidence: In the twos room, the diaper table had several items such as a small bin of toys, a cell phone, and the class attendance record on the surface used for diapering.

Plan of Correction: The non diapering items were removed from the diaper table and will not be stored there.
