Technical Assistance:
Technical Assistance given and Licensing Standards were reviewed regarding allergy lists and fire inspection/ egress doors and steps.

Comments:
An unannounced monitoring inspection was conducted on February 3, 2023. The center closed temporarily from October 14, 2022 and reopened on January 24, 2023. The inspector arrived at 10:15AM and departed at approximately 1PM. There were 8 children present and 4 staff. The Inspector reviewed six children?s records and five staff records. The Inspectors observed staff and children?s interactions throughout the inspection. Children were observed during free play, lunch, and nap. First aid kits and emergency preparedness supplies and document were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented. The violation is listed on the violation notice issued to the center and were reviewed with the center?s director at the exit interview.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, it was determined that the center did not ensure that the areas and equipment of the center be maintained in a safe and operable condition.
Evidence:
1. In the infant room, the garbage lid does not close completely sealed. During the inspection, an infant was able to touch the top of the garbage. The insecure lid poses as a pinching and injury hazard to the children in care.
2. The playground deck?s wooden stair has an approximately 6-8 inch missing piece of wood on one of the steps. There is also a screw sticking out of the missing piece. This poses as a tripping and injury hazard to the children in care.
3. In the preschool room, there is a piece of the baseboard missing from the wall which has chipping paint exposed.
4. In the preschool room, there is an area of the carpet that is starting to fray.
5. Staff #1, the director, confirmed the garbage lid not closing, confirmed the playground deck missing wood, confirmed the missing baseboard, and the frayed carpet.

Plan of Correction: Plan of correction: Staff #1 stated that a work order will be put in for the playground deck, the missing baseboard, and the frayed carpet to have them fixed and repaired. The new garbage can for the infant room has been ordered. Staff will put the current garbage out of reach from the children so that they cannot reach the lid.
