Technical Assistance:
Consultation provided on children with allergies, dietary restrictions and sensitivities.

Comments:
An unannounced monitoring inspection was conducted today with the director. Today is teacher workday and two schools were combined. There were 27 children with 5 staff supervising. There were no medications on site nor had their been any injuries/accidents requiring documentation. Six children and three staff files were reviewed today.
The areas of non compliance is outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on a review of six children's files and interview with staff, it was determined that 3 children did not have immunization record on file at the time of enrollment.
Evidence:
Child B enrolled on 9/18/22, child C enrolled on 8/29/22 and child D enrolled on 8/22/22 and there were no immunization records on file for all 3 children.

Plan of Correction: We will obtain the immunization records.

Standard #: 8VAC20-780-140-A
Description: Based on a review of six children's files and interview with staff, it was determined that 2 children did not have physicals on file within 30 days of enrollment.
Evidence:
Child B enrolled on 9/18/22, child D enrolled on 8/22/22. Neither child had physicals on file.

Plan of Correction: We will obtain the physicals.

Standard #: 8VAC20-780-40-L
Description: Based on observation and interview, it was determined that the center didn't inform all staff of a child's allergy.
Evidence:
Child A has a diagnosed food allergy to red dye, four staff were not aware that this child had this allergy.

Plan of Correction: We will make sure all staff know about what children have allergies.

Standard #: 8VAC20-780-40-M
Description: Based on observation and interview, it was determined that the center didn't maintain a current written list of all children's allergies.
Evidence:
Child A has a diagnosed food allergy to red dye. There was no written list identifying this allergy.

Plan of Correction: We will make an allergy list.

Standard #: 8VAC20-780-60-A
Description: Based on interview it was determined that there were no children's files on site for another program that joined this program on teachers workday. Children's records shall be kept for each child on site.

Plan of Correction: We will make sure to bring copies of children's files.

Standard #: 8VAC20-780-60-A-8
Description: Based on interview and a review of files, it was determined that there was no written care plan for a child with a diagnosed food allergy.
Evidence:
Child A has a diagnosed allergy to red food dye in food. There was no written allergy care plan on file.

Plan of Correction: We will obtain the allergy information.

Standard #: 8VAC20-780-70
Description: Based on interview, it was determined that there were no staff files for 3 staff.
Evidence:
There were no staff files on site for staff A, B and C.

Plan of Correction: We will make sure staff files are always present with staff are on site.
