1. The record for staff #5 (hired 1/30/23) did not contain documentation of the required negative tuberculosis screening before hire.

Standard #: 8VAC20-780-260-A Description: Based on record review it was determined the center could not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.
Evidence:
1. Director reported the inspection was completed in January 2023 but did not have the inspection report.
2. Director showed inspector the updated fire permit dated 1/2023.Plan of Correction: Director will request a copy of the fire inspection.
Standard #: 8VAC20-780-270-A Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
In the outdoor play area there was:
- Peeling paint on the red activity center and yellow picnic table
- Protruding screw head and metal bracket on the rec activity centerPlan of Correction: Director will ensure all equipment in is good repair.
Standard #: 8VAC20-780-280-B Description: Based on observation and interview it was determined the center did not ensure that all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. There was sanitizing solution in the two younger toddler classrooms on an open upper shelf out of the reach of children.
2. Staff confirmed that the sanitizing solution was unlocked.Plan of Correction: All chemicals will be locked.
