Comments:
An unannounced renewal inspection was conducted on-site February 1, 2023 and concluded remotely February 6, 2023. The director was available during the inspection. There were 19 children present, ranging in ages from 3 years to 5 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 6 staff records, 4 board member records, and 1 agent record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-770-40-D-1-a
Description: Based on a review of records on February 1, 2023, the center failed to ensure to obtain a central registry check from each Officer of the Board before the end of the 30 days for each new officer after the change of officer.
Evidence: 1. The record of Officer #1, took office 7/14/22, contained documentation of central registry results dated 8/22/22.

Standard #: 8VAC20-780-150-B
Description: Based on a review of children's records and interview on February 1, 2023, the center failed to ensure that immunizations received shall be signed by a physician, his designee, or an official of the local health department.
Evidence: 1. The records of child #1 and child #5 contained documentation of immunizations that were not signed by a physician, his designee, or an official of the local health department. 2. Staff #4 confirmed the immunizations were not signed.

Standard #: 8VAC20-780-70
Description: Based on a review of records and interview on February 1, 2023, the center failed to keep information at the center about any health problems that may interfere with fulfilling the job responsibilities for each staff.
Evidence: 1. There was no documentation at the center of information about any health problems that may interfere with fulfilling job responsibilities for staff #1, staff #2, staff #3, staff #4, staff #5, and staff #6. 2. Staff #4 confirmed that human resources was informed about needing the information at the center and said that information is still not kept at the center.
