A monitoring inspection was conducted on 02/17/2023. A tour of the facility was conducted, in addition to a record review and interviews with staff. The inspector emailed the director/provider a list of items required to complete the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the

Standard #: 8VAC20-780-270-A Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe and operable condition.
Evidence:
The handle to one of the drawers in the kitchen was broken.Plan of Correction: This will be reported to the owner for repair.
Standard #: 8VAC20-780-280-B Description: Based on observation the center did not ensure all Hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
There was an unlocked bottle of air deodorizer on top of the towel dispenser in the bathroom.Plan of Correction: A parent left it in the bathroom. It was removed and placed in a locked area.
