Comments:
A renewal inspection was initiated on February 15, 2023 and concluded on February 16, 2023. There were fourteen children present, ranging in ages from four weeks to five years, with three staff supervising. Additional staff arrived during inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on review, facility did not ensure 1 of 5 staff have an Office of Background Investigations (OBI) fingerprinting letter of eligibility prior to beginning employment.

Evidence: The record for staff #4 (start date 2/13/2023) did not have an OBI fingerprinting letter of eligibility. Owner confirmed the individual did not have a letter of eligibility for this facility.

Evidence: The following was found during record review:
1. The record for child #4 (start date 11/29/2022) did not have a parent employment phone number, physician name, physician phone number, school currently attending and programs previously attended.
2. There was not a record available for child #5, who was present during inspection.
3. Staff #1 verified information was missing from the children?s records.

Description: Based on observation, facility did not ensure areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

Evidence:
1. In the infant room, trash and debris were located on the floor of the crib area. The trash and debris included a small plastic bag of pretzels, a cloth mask, crumpled and torn papers, magic marker, pen, and small particles of dirt/trash. This area bridges the two rooms used for infant care. One child was observed mobile crawling and walking. These items were accessible to children in care.
2. On the outside playground, a shed on the paved and fenced in area had broken wood along the bottom of the shed door leaving splintered wood and an opening to a potentially hazardous area. The shed is accessible to children in care when on the playground.
3. In the toddler room, a 3? by 4? bulletin board was propped against the wall. The bulletin board was not secured and could be pulled over on a child.

Description: Based on observation, facility did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: Disinfectant spray and alcohol wipes were stored on an open bookcase in the toddler room. Both items were labeled ?Caution and keep out of reach of children?.

Description: Based on observation and interview, facility did not ensure when hazardous substances are not kept in the original containers, the substitute container is labeled with its contents.

Evidence: A spray bottle with a yellow liquid was stored on a bookcase in the toddler room. Staff #1 stated she did not know what was in the bottle and asked Staff #4. Staff #4 stated it was ?cleaner Lysol?.

Description: Based on observation, facility did not ensure children under 10 years of age are within actual sight and sound supervision of staff.

Evidence: On February 15, 2023 at approximately 9:40 AM, staff # 2 was observed leaving 5 children, ages 4 and 5 years, alone in the classroom to take a child to another classroom. The children were not in sight supervision of a staff during this time.

Description: Based on interview, facility did not ensure there is at least one staff for four children when the youngest in the group is of infant age (birth up to sixteen months).

Evidence: Staff #5 stated she was alone in the 2?s room with six children on this date before staff #1 and staff #2 arrived at 8:00 AM. Staff #5 stated Staff #4 was present in the building, but she was working in another room. Staff #5 stated child #2 (age 1 month) was the youngest in the group.

Description: Based on observation and interview, facility did not ensure cribs, cots, rest mats and beds are identified for use by a specific child.

Evidence: Staff stated 2 cribs are currently assigned to children however the cribs were labeled with different children?s names. Staff #1 stated the crib labels have not been updated.

Description: Based on observation and interview, facility did not ensure children?s hands are washed with soap and running water or disposable wipes before and after eating meals and snacks.

Evidence: In the infant room, two older infants were observed sitting on the floor eating puff snacks. Staff #1 stated Staff #4 had washed the children?s hands earlier however children had been observed crawling and sitting on the floor just prior to eating.
