Comments:
An unannounced renewal inspection was conducted on 2/14/23 from 10:00am to 11:35am with the Assistant to the Head of School and program staff. There were 11 children in care, ranging in age from four-years to five-years-old, supervised by two staff. The children were in music class and French class during the inspection. Four child records, two staff records, and four Board Officer records were reviewed. Both classroom staff have current certification in CPR and First Aid, Daily Health Observation Training, and Medication Administration Training (MAT). There were no medications for children on-site today. The attendance and emergency drill log were reviewed. Required postings, the first aid kit, flashlight, and battery-operated radio were observed. Standard #: 22.1-289.036-B-2
Description: Based on review of four Board Officer and two Agent records, the center did not obtain the results of a national, fingerprint-based criminal history check within 30 days of beginning service for one Board Officer. Evidence: The fingerprints on record for Board Officer #1, who began service on 7/1/22, were dated 3/16/23.

Plan of Correction: The Board Officer was made aware of this requirement over the Summer but they did complete the background checks until now unfortunately. Moving forward, we will continue to stress to new Board Officers that these checks are required and are very important to our License.

Standard #: 8VAC20-770-40-D-1-a
Description: Based on review of four Board Officer and two Agent records, the center did not ensure that a Board Officer submitted a sworn statement within 30 days of beginning service or that the results of a search of the central registry were obtained for the Board Officer within 30 days of beginning service. Evidence: The sworn statement submitted by the center for Board Officer #1, who began service on 7/1/22, was dated 3/13/23. The center reported that the central registry check for the Board Officer was mailed on 3/17/23.
