An unannounced on-site renewal inspection was conducted on 02/28/2023. There were 29 children, ages 5 -12 years, and 3 staff members present during the inspection. The inspector observed the following: free choice activities, arrival and bathroom time. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: program director and applying for an AV. The inspector arrived at 6L50 AM and departed at 8 AM.

Standard #: 8VAC20-780-130-A Description: Based on review of five childrens records, the center failed to ensure that all children had the immunization record prior to beginning attendance.
Evidence:
The record for Child # 4, with 08/16/2022 as the first day of enrollment , did not have the immunization record on the day of the inspection.Plan of Correction: The program director will obtain the missing immunization from the parent.
Standard #: 8VAC20-780-140-A Description: Based on review of five children's records, the center failed to ensure that all children had a physical examination within 30 days of enrollment.
Evidence:
1. The record for child # 1 with 08/16/2022 as the first day of enrollment did not have a documented physical examination on the day of the inspection.Plan of Correction: The Program Director will obtain the missing Physical from the parent.
