An unannounced complaint inspection was conducted on 1/11/2023. The information gathered during this inspection determined the complaint to be valid. Non-compliance with applicable standards or law and violations are documented on the violation notice issued to the provider. The inspectors arrived for the inspection at 12:53 pm. and departed at 1:30 pm.

Description: Based on review of two children?s records, the center failed to ensure that all records were complete per the standard.

Evidence:
1.The record for Child # 1, who started on 3/1/2021, was missing the address for one of the two emergency contacts.
2.The record for Child # 2, who started on 8/11/2021, was missing the address to the two emergency contacts.

Plan of Correction: Children's records will be fixed and will be monitored.

Description: Based on observation and interview with staff, the center failed to ensure that during designated rest period, when children are resting or in an inactive state, the following rest period ratios are permitted: Children two years of age: one staff per 16 children.

Evidence:
Observed 29 children resting on cots in the center?s nap area. The children were aged 2 years old to 5 years old and were under the supervision of 1 staff member. The ratio is 1 staff to 16 children when the group has 2 year old children. Two staff members would be required.

Plan of Correction: Two staff at all times during nap.

Description: Based on observation of the center?s nap area, the center failed to ensure that all occupied cots had at least 12 inches of space between each one.

Evidence:
Observed 29 children resting on cots in the napping area. There were multiple children on cots that were directly touching other children?s cots. Staff # 1 confirmed that this has been occurring for a couple weeks since Winter break.

Plan of Correction: Nap has been moved back to the large opened classroom to ensure that the required amount of space for each cot is met.
