Technical Assistance:
Technical Assistance was discussed with the Director and Area Manager regarding:
-the assessments used to determine child development/readiness for the next class in relation to the standards and ratios
-the menu and snacks
-cots and linens

Comments:
An unannounced renewal inspection was conducted on 3/1/23 from 9:45am to 11:50am with the Director. There were 31 children in care, ranging in age from nine-months to five-years-old, supervised by five staff. Infants were observed being fed, napping, and playing on the floor with staff; older children were observed on the playground and transitioning inside for snack. Five child records and four staff records were reviewed. All of the center staff have current certification in CPR and First Aid, and three staff are trained in Daily Health Observation Training. Five medications and authorization forms were reviewed and the center has one staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. The most recent Fire Inspection on file was dated 3/17/22 and the most recent Health Inspection on file was dated 4/1/22. Morning snack served was goldfish and juice. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 3/6/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of documentation, the center did not ensure that each staff member submitted documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Evidence: The record for Staff #1, with an employment date of 6/6/22, contained documentation of a TB screening dated 6/13/22.

Plan of Correction: Staff member was hired during covid to make sure center was complying to staff-to-child ratios. During the Nation-Wide pandemic, the Health Department stopped offering TB screening clinics and switched to appointment only. Appointments were being booked several weeks out. Employee was hired and received her TB screening as soon as possible. Employee received negative TB results at that time.

Standard #: 8VAC20-780-350-B-4
Description: Based on observation, the center did not ensure that the required staff to child ratios were followed. Evidence: On 3/1/23 at approximately 9:56am one staff was observed supervising 13 children aged three- to five-years-old. The required ratio was one staff to 10 children for that age group.

Plan of Correction: The childcare industry is still facing a massive staffing shortage post covid.

We are actively trying to hire additional staff that meets all qualifications.

Standard #: 8VAC20-780-350-C
Description: Based on observation, interview, and review of documentation, the center did not ensure that when children are in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest child in the group applied to the entire group. Evidence: On 3/1/23 13 children were observed in the ?Twos/Threes? class with two staff. Interview revealed that there are two children in the class who are 17-months-old and 21-months-old. With children 17-months-old and 21-months-old in the class, the required ratio was 1:5.

Plan of Correction: The 17-month-old and 21-month-old in question had an assessment form on file signed by parents, director, and teachers. This form has been approved for 20 years by previous inspectors. It is still being used at 2 of our other centers with no questions from those inspectors. Our inspector requires an additional form on how we assess the child to be placed in an older group of children. We will create a more detailed assessment form to be attached to our original assessment form.
