The licensing inspector initiated an unannounced inspection on 02/22/23 and concluded on 02/22/23 from 10:40 AM to 11:55 AM in response to a complaint received by the department on 02/09/22 for allegations regarding supervision, ratios, emergency response and training. Interviews, record review and observations were conducted relating to the allegations. The information gathered during the investigation does not support the allegation, so the complaint is determined to be "not valid."
