Technical Assistance:
First aid and cardiopulmonary resuscitation certificate that includes an in-person component was discussed with staff 2.

Comments:
A monitoring/renewal/complaint inspection was conducted on 03/22/2023. There were 24 children present, ranging in ages from 2 to 4 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with staff 2 during the exit interview.

Description: Based upon observation, the facility has not ensured that they maintain a current written list of all children's allergies, sensitivities and dietary restrictions that is dated and kept confidential.
Evidence:
There was an allergy chart posted on the wall of the preschool classroom. The chart was not covered or made confidential. The children's names and allergies were open to viewing by whomever entered the room. The list was not dated.

Description: Based upon observation, the facility has not ensured that equipment, inside and outside, is maintained in a safe condition.
Evidence:
The metal handle bars on a tricycle and on a toddler balance bike on the playground were not covered with handle bar grips, creating a potential impalement hazard should a child fall on the handle bars.

Description: Based upon observation and staff interview, the facility has not ensured that when hazardous substances are not kept in the original containers, the substitute containers clearly indicated their contents.
Evidence:
Staff 4 was observed cleaning the changing table with a fluid in a spray bottle. The bottle was not labeled as to the contents. Staff 4 acknowledged that the spray bottle was not labeled and stated that it was "Fabuloso" multi purpose cleaner in the spray bottle.

Description: Based upon observation, the facility has not ensured that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 as shown in Figures 2 (Compressed Loose Fill Synthetic Materials Depth Chart) and 3 (Use Zones for Equipment) on pages 6-7 of the National Program for Playground Safety's "Selecting Playground Surface Materials: Selecting the Best Surface Material for Your Playground," February 2004, and shall be under equipment with climbing apparatus to create a fall zone free of hazardous obstacles.

The playground has a large climbing structure. There was less than 1/2 inch of resilient surfacing in the entire fall zone of the structure..

Description: Based upon observation, the facility has not ensured that soiled diapers are disposed in a lidded diaper pail that is operated in such a way that neither the caregiver's hands nor the soiled diaper touch the outside of the diaper pail.
Evidence:
Staff 4 was observed changing a child's diaper after outdoor play. Staff 4 used her hands to open the lid on the diaper pail and dispose of the soiled diaper.

Evidence:
The documentation of emergency drills did not include the following:

5. weather conditions for evacuation drills.
