Comments:
A monitoring inspection was conducted on February 27, 2023 from approximately 12:45 PM to 1:55 PM. There were 14 children present in the center with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A member of management stated the center does not administer medications and transportation is not provided. A total of five child records and three staff records were reviewed.


Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Description: Based on record review and interview, one staff record did not have documentation of an updated sworn statement every five years from the date of the most recent sworn statement.
Evidence:
1. The record for staff #3 did not have an updated sworn statement every five years from the most recent Sworn Statement. Staff #3 had a sworn statement dated 11/08/17.
2. An administrator stated that the Sworn Statement had not been updated.

Plan of Correction: Corrected while inspector was on site.
