Comments:

A monitoring inspection was conducted on February 28, 2023 from approximately 8:35 AM to 1:00 PM. There were 99 children present in the center, ranging in ages from infants to pre-k, with 24 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Medication is administered, and medication, authorization forms and allergy plans were reviewed. The provider stated that transportation is provided, and the vehicle and transportation documents were reviewed. A total of ten child records and ten staff records were reviewed. Health Inspection: 3/16/2022 Fire Inspection: 12/05/2022 Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1.In room #3, two window screens are broken. In room #8, one broken screen. In room 10, one broken screen. 2.In room #4, one kitchen set had a broken board at the corner of the kitchen set exposing a sharp wood edge.
Plan of Correction:	The kitchen set is removed from the classroom.

Description:	Based on record review, the licensee did not ensure that there was a minimum of one lockdown drill practiced, yearly. Evidence: The facility did not have documentation of a lockdown drill for 2022.
Plan of Correction:	We will conduct a lock down drill once a year going forward.
